Insurances Accepted
Condo Pharmacy provides pharmacy benefits for many different insurance companies. We process claims for most Medicare Part D plans, the New York State Medicaid program, the New York State EPIC program, the New York State ADAP Program, and the New York State Prescription Saver Program. Condo Pharmacy also processes prescriptions for the Clinton County Prescription Saver Program.
Condo Pharmacy will attempt to process worker’s compensation claims as well as no fault claims but cannot and will not make any guarantees as to whether the claims will be approved or denied in advance.
Additionally, some employers have entered into restricted network contracts for prescription benefits. For questions about network pharmacies, refer to the member services department of your insurance company.
The list that follows is current as of its posting, but is subject to change and often does. For that reason, and because of possible differences between the names below and the name that may be on a prescription benefit card, we encourage you to call for confirmation.
