Delivery Services

 Free delivery of prescription medication pursuant to the following:

• One trip daily per household, two trips weekly.
• Durable Medical Equipment, Over the Counter Medication, Vitamins, and other store stock may be added to any prescription order free of charge.

Minimum delivery order (non prescription): $10.00
 
Any delivery order not meeting the above requirements is subject to a $3.00 gas and service charge.
 
In order to provide the best possible service to our patients any orders placed one day will be delivered the next delivery day.

• Refill requests may be placed up to seven days early under most insurance plans and for most prescriptions.
• Controlled substances by law cannot, and therefore, will not, be filled until they are due.
• Every attempt to accommodate special circumstances such as antibiotic orders and patients facing same day discharge from a hospital or rehabilitation facility will be made.

 Delivery service is available within the greater Plattsburgh area from 10:00 AM to 4:00 PM Monday through Friday.

• Morning or afternoon delivery may be requested but a specific time cannot be guaranteed.
• Any delivery that cannot be left because no one is home will be brought back to the pharmacy at the end of the day and remain at the pharmacy for pick up until alternate arrangements are made by the patient.
• We will not attempt to redeliver any medication or item on the same day.

Prescriptions may be picked up from your home, office, or prescriber and delivered the next delivery day.

• Ask your prescriber to fax any prescription(s) to us and we can pick up the originals from you when we deliver your medication.
• We will not deliver controlled medication until the original prescription is in our possession.
• Additionally, our software is capable of receiving electronic prescriptions directly from your prescriber.

Unless prior arrangements have been made, or you have a store charge account in good standing, payment, including any prescription co-pays, is due at the time of delivery. For your convenience, checks and most major credit cards are accepted.

• Credit card information must be provided prior to delivery.
• For your protection, identification will be required of any unknown persons at the time of delivery.

Special Shipping:
Assuming product stability can be assured, Condo Pharmacy will ship any prescription or special order using a trusted carrier.  As per state and federal law, as well as the rules and regulations set forth by all insurance companies, a signature is required upon the delivery of all prescriptions.  All additional shipping charges, including compliance with the signature requirement, are the responsibility of the patient.  
 
Despite reimbursement cuts by all insurance companies and the increased cost of gasoline, we will endeavor to keep our prescription delivery service free of charge to all of our patients through policies and procedures such as those outlined above, we appreciate your cooperation and look forward to serving you!
